– Savings and Checking account numbers and routing number

Tax Forms
– INTAKE FORM - print this and complete pages 1-3
– A copy of your 2016 tax return
– W-2 from all jobs held during the 2017 year
– 1095-A (Health Insurance Information)
– 1095-B (Health Insurance Information)
– 1095-C (Health Insurance Information)
– 1099-G (unemployment)
– 1099-INT (bank interest)
– 1099-MISC and business expenses
– W-2G (gambling)
– 1098 (dividend or interest)
– Alimony Income
– Retirement income and contributions
– Social Security Benefits or S.S.I.
– Information regarding education expenses, including student loan interest, tuition, or other student fees paid during the year
– Mortgage interest paid, real estate taxes paid, charitable contributions, and medical expenses
– Unreimbursed childcare expenses, paid provider's name, address, and SSN or tax ID number
– Any other tax documents mailed to you
